For Each item in the left column, one of the three column will be selected! I may increase the number of rows and columns if required.

After collecting the responses from the audience, I need to produce a wrap up report containing graphical output representing the audiences preference for each item (written in left column on the basis of choices defined in the right columns.

Our surveys can be created in all paid plans of Slido. 

I assume it is not exactly as you wanted but this would be as sort of workaround for your use-case.
