How do I get the participant to enter his name, email and phone number as well. I don’t see the phone number as an option.

We don’t have an option to add a mobile field, but you can definitely share this idea in our Wishlist where our product team can see the demand and you can track updates on its status.
The only workaround we have is our paid advanced customization where we can rename a field to phone number for you.
