I think it is very helpful to put a different pictue in every option of a multiple choice question.
e.g.
Question: Which animal is the one that barks?
Option A: Cat (with a picture of cat)
Option B: Dog (with a picture of dog)
Option C: Hamster (with a picture of hamster)
